Patients and their families know us for our personalized, high-quality care, delivered with empathy, kindness, and respect.TITLE: Nurse – RN & LVNOFFICE LOCATION: Ukiah CAPATIENT TERRITORY: Lake & Inland Mendocino CountiesSCHEDULE: Flexible hour coordinated with the Director of Patient Care ServicesPOSITION SUMMARY: In coordination and collaboration with the DOPCS· Provides accurate assessment, over-sight, and monitoring of patients for quality medical management and early detection of changes in condition.· Responds promptly to evaluate and remedy patient concerns and complaints; manages anddocuments accordingly.· Utilizes professional standards in performing basic assessment and clinical monitoring in accordance with scope of licensure.· Recognizes AND appropriately responds to emergent and significant change in condition; completes documentation as required.· Effectively communicates accurate assessment detail, medical history, advanced directives.· Encourages Patients to participate in prescribed therapies, dietary regimen, and activity programs, as appropriate.AREAS OF RESPONSIBILITY: Performs an initial, comprehensive assessment.Documents observations, clinical findings, problems, skilled interventions, goals and discharge plansIn consultation with the DOPCS, initiates and regularly re-evaluates and revises the plan of care.Assesses the need for the services of other team members.Provides and documents skilled care on all visits (includes skilled observation of the patient’s condition, skilled care, and procedures and teaching of the patient and/or family).Obtains and documents physician orders.Performs Home Health Aide supervisory visits per regulatory requirements.Coordinates care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient’s condition and needs. Documents these communications.Submits weekly visit schedule of assigned patients. Collaborates with Nursing Scheduler to address scheduling needs.Performs re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager. Completes and submits all related documentation.Attends and actively participates in the clinical team multidisciplinary patient conference.Demonstrates established clinical competencies.Participates in agency sponsored in-service education.Participates in quality improvement activities.Assesses, develops, organizes and delivers teaching materials for assigned home care patients as appropriate.Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.Communicates patient’s needs and any changes to the interdisciplinary groupProvides and maintains a safe environment for the patientPHYSICAL/MENTAL REQUIREMENTS· Mobility-Constant walking; frequent driving; Lifting-Frequent lifting of less than 5 to 30 pounds; Visual Requirements-Constant overall vision, eye-hand coordination; frequent color perception, reading/close-up work, field of vision/peripheral; Dexterity-Constant writing; frequent holding, reaching, grasping, and repetitive motion; Hearing/Talking-Constant hearing normal speech, talking in person, talking on the telephone; frequent hearing on the telephone; Emotional/Psychological Factors-Constant public contact, decision making, concentration; frequent exposure to trauma,grief, death, working alone; Environmental Conditions-Constant exposure to noise; frequent exposure to dirt, dust, smoke, cold,/heat (indoor and outdoor).TYPICAL WORKING CONDITIONSWork is performed home environments; exposure to communicable diseases and blood borne pathogens; continuous contact with other staff and the public. Incumbent are expected to follow universal precautions.
